Fulcrum Tuition Assistance Application for 2008-2009

To obtain a tuition assistance grant at a participating Catholic school in the archdiocese, the family must complete this confidential application form and submit a printed copy of it with copies of the required financial documentation to the school where the family member(s) will be attending. The principal of the school forwards to the Fulcrum Foundation all completed applications. If you are unable to complete the form online, contact your school for a printed application form that can be filled out by hand.

Online Application Procedure

  1. Be sure your computer is connected to a working printer. You can test this by pressing the print button below and printing these instructions for future reference.
  2. Make a copy of your 2006 Federal Income Tax Form(s), IRS 1040, 1040A or 1040EZ, for the financially responsible parent(s) or guardian(s). Note: W-2 forms are not acceptable. Do not send originals.
  3. Make a copy of any documents related to Non-Taxable income (Child Support, Welfare, Social Security) and any additional 2006 Federal Tax Schedules A-F and 1099, if applicable.
  4. Enter online all the data requested in the application form. When completed, press Submit to send the data.
  5. Print two copies of the completed form, this can be done immediatly after submitting the form.
  6. Sign the form in Part E.
  7. Staple the signed application in front of copies of the financial documents from step 2 and 3.
  8. Deliver or mail the complete application package to the designated school before the deadline of your designated school. Copies will not be returned.
  9. If you have students in both a Grade school(K-8) and a High School(9-12), repeat this process for the other school.
