A student applying to multiple schools need only apply for assistance through Fulrum Foundation for one school. Upon enrollment the assistance grant can be transferred to any other Archdiocese school. Elementary and Secondary students must apply separately.

Part C Parent Income and Asset Information

1. Taxable Income. Enter your total 2006 earned wages, salaries and tips. If filing jointly, the sum is equal to line 7 on IRS form 1040/1040A, or line 1 on 1040EZ. If parents filed separately, enter separate amounts.	Your 2006 earned income

2. Adjusted Gross Income. Enter the adjusted gross income reported on the 2006 form 1040-line 37, 1040A-line 21, or 1040EZ-line 4. If parents filed separately, enter the sum of the two adjusted gross income amounts.
3. Non-Taxable Income. ( Child Support + Welfare (ADC) + Social Security + Other non-taxable income ) Enter the ANNUAL amount received in 2006 for each category for the entire family. Attach copies of documents from the relevant state or federal agencies. Add the four items and enter the total NTI.
4. Tax Paid. Enter the total tax paid as shown on the 2006 IRS form 1040-line 63, 1040A-line 38, or 1040EZ-line 10. If parents filed separately, enter the sum of the two tax paid amounts.
5. Cash Assets. Enter the current cash value of all cash, savings and checking accounts for the entire family. (If under $1000 enter zero.)
6. Family Residence Equity. ( Present Assessed Value - Unpaid Mortgage ) If you own a home, enter the difference of the Present Assessed Value and Unpaid Mortgage. If renting, enter zero.
7. Other Real Estate and Investments. ( Present Assessed Value - Unpaid Mortgage/Liability ) If you own other real estate or investments, enter the difference of the Present Assessed Value and Unpaid Mortgage or liabilities. If none, enter zero.
8. Business/Farm. ( Present Market Value - Unpaid Mortgage/Liability ) If you own a business/farm, enter the difference of the Present Market Value and Unpaid Mortgage or liabilities. If none, enter zero.
